GUIDELINES for VETERANS AID & ATTENDANCE BENEFIT *The Department of Veterans Affairs may need additional information. This is just a guide put together to help you get a head start. It is not an official document. For more information, please contact Luther Ridge's Director of Admissions or Brenda Zechman, Veterans Affairs Director for Schuylkill County. She is located at the Schuylkill County Courthouse. She can be contacted by phone at (570) 628-1400. Information you need for Veterans' Aid & Attendance benefit application: What you need to know about the benefit: DD214 (discharge papers - original or certified with seal only) Death certificate (for widows) - original Marriage license - not certificate Documentation regarding stocks, bonds, CDs, etc. Documentation regarding bank statements: checking, savings, etc. Copy of End-of-Year Social Security Statement or other proof of annual income Out-of-pocket medical bills (ask your pharmacy, doctor's office, or medical center to print out a financial statement covering the past year for you) Statement of cost for insurance supplement (Blue Cross or Blue Shield, for example) Medical statement for consideration of Veterans' Aid & Attendance - Form 21-2680 You must be 65 or older This benefit is for veterans, their spouses, and widows Must be a wartime veteran Must be in assisted living Must have been honorably discharged Must have served at least 90 days The qualifying guideline is $120,000 There are also income guidelines, which you will need to discuss with the Veterans' Administration The application process may take 9 to 12 months This benefit is need based.
